What You'll Be Doing

n

n
- Partnering with NDIS participants to implement their plans effectively
n
- Assisting clients to understand their funded supports and make informed choices
n
- Coordinating a network of supports, including providers, health professionals, and informal networks
n
- Advocating for participant needs and ensuring services align with their goals
n
- Keeping thorough documentation and maintaining compliance with NDIS requirements
n
- Identifying and managing risks, and supporting participants through plan reviews
n




- Staying current with NDIS practices and changes
n
- Participating in reflective practice, supervision, and team development activities
n

What You Bring to the Role

n

n
- Experience in Support Coordination or a related support role
n
- Knowledge of the NDIS and a strong commitment to the rights and goals of people with disability
n
- A qualification in Social Work, Psychology, Allied Health, or Community Services (or similar)
n
- Strong time management and communication skills
n
- Ability to work independently and as part of a team
n
- Capacity to manage a caseload and travel within Brisbane, Gold Coast, Ipswich & surrounding areas
n
- Valid Driver's Licence and reliable, insured vehicle
n
- Current First Aid and CPR
n
- NDIS Worker Screening Check & National Criminal History Check (or willingness to obtain)
