CQ Medicentre South is seeking an experienced Chronic Condition Management Nurse to join our established and highly regarded practice in Rockhampton.

About the Role

This position has a strong focus on Chronic Condition Management and preventative healthcare initiatives.

Responsibilities include:

- Coordinating Chronic Condition Management plans
- Conducting Care Plan Reviews
- Completing Health Assessments
- Patient education and health coaching
- Recall and reminder management
- Working collaboratively with GPs and allied health providers




- Monitoring patient outcomes and follow‑up activities
- Maintaining accurate clinical documentation

About You

We are seeking someone who is passionate about patient‑centred care and enjoys helping patients achieve better long‑term health outcomes.

You will have:

- Current AHPRA registration as a Registered or Enrolled Nurse
- Current CPR certification
- Experience in General Practice nursing
- Demonstrated experience with or the ability to learn: Chronic Condition Management Plans, Care Plan Reviews, Health Assessments, Patient follow‑up programs.
- Excellent organisational and communication skills
- Full Australian working rights
- Experience using Best Practice software will be highly regarded.
