Job Description

Mounties Care is seeking a dedicated Home Care Partner to support clients in their homes and community, three days per week at our Dandenong office. You will coordinate care delivery, oversee carers and ensure quality services, while building client plans and budgets.

The role requires solid communication, 2–3 years of relevant experience, and current First Aid/CPR with a National Police Check. You will collaborate with clients and families, access My Aged Care, and participate in after-hours
