Blossoming Support Service is a community-based care provider committed to delivering respectful, reliable and person-centred support to clients in their homes and in the community.

Due to increasing demand for our services, we are seeking an experienced and compassionate Aged or Disabled Carer to join our team on a full time basis.

n

Key responsibilities

n

- Provide personal care, including assistance with showering, dressing, grooming and toileting
n
- Assist clients with mobility, transfers and the safe use of mobility equipment
n
- Provide companionship, emotional support and reassurance
n
- Assist with meal preparation, feeding, nutrition and hydration
n
- Perform light household duties, including cleaning, laundry and changing bed linen
n
- Support clients with shopping, appointments, recreational activities and community participation
n
- Provide medication assistance or reminders in accordance with care plans and company procedures
n
- Follow individual care plans, behaviour support plans and risk-management procedures
n
- Observe and report changes in clients' health, wellbeing, behaviour or support needs
n
- Maintain accurate progress notes, service records and incident reports
n
- Promote client choice, independence, privacy,



dignity and cultural safety
n
- Follow infection control, manual handling, WHS and safeguarding procedures
n
- Communicate effectively with clients, families, care coordinators and healthcare professionals
n

Qualifications and experience

n

n
- Certificate III in Individual Support (Ageing or Disability), or an equivalent relevant qualification
n
- Alternatively, substantial relevant experience equivalent to the required qualification
n
- At least 1 year of relevant experience as an aged care worker, disability support worker, personal carer or similar
n
- Current First Aid and CPR certificates
n
- Current National Police Check
n
- NDIS Worker Screening Check where required
n
- Valid Australian driver licence and access to a reliable insured vehicle where community travel is required
n
- Strong communication, documentation and interpersonal skills
n
- Ability to work independently and as part of a multidisciplinary team
n

Full-time position – 38 hours per week
n Salary: $79,500 per annum + superannuation

n

Availability to work on rotating rosters, including some evenings, weekends and public holidays, may be required.
