The Role

n

The Palliative Care Program (PCP) provides proactive, client‐centred care to individuals with advanced chronic illnesses and life‐limiting conditions. Referrals to the program should be made as early as possible following diagnosis.

n

Palliative Care:

n

n
- Affirms life and recognises dying as a normal process
n
- Enhances quality of life and may positively influence the course of illness
n
- Neither hastens nor postpones death
n
- Advocates for relief from pain and other distressing symptoms
n
- Integrates psychological and spiritual aspects into client care
n
- Supports clients to live as actively as possible until death
n
- Offers support to families and carers during the client's illness and throughout bereavement
n




- Uses a multidisciplinary team approach to meet client and family needs, including bereavement counselling where appropriate
n
- Is applicable early in the illness trajectory, alongside curative or life‐prolonging therapies.
n

Key Selection Criteria

n

Mandatory

n

n
- Current registration with AHPRA as a Registered Nurse.
n
- Current, unsuspended and unrestricted drivers license.
n
- Demonstrated understanding of the term "Palliative Care".
n
- Highly developed, demonstrated skills and recent experience in providing Palliative Care to clients and carers.
n
- Highly developed interpersonal and communication skills.
n
- Demonstrated experience communicating effectively with persons from diverse backgrounds and cultures.
n
- Understanding of how the issue of privacy and confidentiality relate to client care.
n
- Ability to work across a seven day week roster.
n
- Computer skills, particularly the Microsoft Office Suite.

PLEASE NOTE: SCHS is committed to the application of the Child Safe Standards. All positions are subject to thorough credentialing and safety screening prior to commencing employment. This includes reference checks and National Police Check for all SCHS employees. Any prospective employee who has resided and worked overseas in the past 10 years for a continuous period of 12 months or more will be required to undergo an International Criminal History Check. Where applicable, employees may also be required to hold a current Working With Children check issued in the state of Victoria and have a current NDIS Clearance (NDIS Worker Screening Check).

Department of Health Guidelines for Immunisation of Health Care Workers require all staff to demonstrate immunity to specific preventable diseases. As part of the recruitment process, the preferred applicant is required to provide evidence of immunisation (and/or serology) to confirm they are protected from risk.
