UnitingCare Queensland is seeking a Supported Living Facilitator to visit clients in their homes, providing daily living support and personal care. You’ll travel between client locations, helping with meals, hygiene, and household tasks while following individualized care plans.

You will work mainly business hours, Monday to Friday, with travel across Plainland and surrounding areas. A current driver’s licence, CPR/First Aid, and NDIS screening are required.
