Wesley Mission is seeking a passionate Case Manager to join the Intensive Therapeutic Care – Significant Disability (ITC-SD) team, delivering trauma-informed, person-centred support for children and young people with significant disabilities.

You will lead permanency planning, develop and review case plans, collaborate with therapeutic specialists, house managers and support workers, and ensure OOHC policy compliance while pursuing positive outcomes.
