Mounties Care Home care is seeking a dedicated Care Partner to join our team on a full time basis at our Hope Island, QLD office. You will work with our clients to help them stay as independent as possible at home and in the community, coordinating care and services with a trusted team of Carers to ensure quality delivery.
Key duties include developing client care plans, preparing budgets, assisting access to My Aged Care, visiting clients in the community, and supporting potential client leads
