Duties:

n

n
- Conduct triage, comprehensive assessments, risk assessment, and safety planning.
n
- Deliver short-term case management and assertive outreach.
n
- Provide brief psychoeducation and therapeutic interventions.
n
- Collaborate in care planning and discharge with internal and external stakeholders.
n
- Practice in accordance with Trauma-Informed Care and Recovery principles
n

n

The successful canidate will possess:

n

n
- Relevant qualification in Social Work.
n
- Eigibility for membership with the AASW.
n
- Experience in mental health clinical or community settings.
n
- Strong assessment and case management skills.
n
- Experience with CIMHA database preffered.
n
- Catagory A immunisations.
n
