Ramsay Clinic Wentworthville is seeking a Care Navigator – Recovery Support/Outreach to coordinate post-discharge outreach for outpatients as part of a multidisciplinary team in a permanent full time capacity.

You will engage patients across age groups with various mental health diagnoses, delivering follow-up care aligned to individual plans, while working under the Director of Clinical Services to meet departmental goals.
