Your role as a Service Navigator – Ageing Well, you'll play a pivotal role in helping older Australians and their families access the support they need to live well and age with confidence. As the first point of contact for our services, you will deliver an exceptional customer experience through intake, assessment, service navigation and onboarding, ensuring each client receives personalised guidance tailored to their individual needs and goals.

Working within a client-centred and values-driven environment, you'll support the delivery of high-quality aged care services that empower clients to maintain independence, wellbeing and quality of life. You'll also build strong relationships with internal teams, healthcare professionals and community partners to facilitate seamless transitions into and across Wesley Mission Queensland services.

What We’re Looking For

- Deliver exceptional customer engagement by providing information, advice and support to prospective clients and their advocates regarding aged care and community services.
- Manage the full client acquisition journey, from lead generation and nurturing through to onboarding, ensuring service growth targets and KPIs are achieved.
- Conduct client assessments to identify needs, preferences and risks, recommending appropriate services and funding options.
- Prepare and manage onboarding documentation, including service agreements, fee estimates and legislative requirements.
- Build and maintain strong relationships with referral partners, healthcare professionals and community stakeholders to generate referrals and business opportunities.
- Promote services through networking, industry events and community engagement activities.
- Maintain current knowledge of aged care funding, legislation and service offerings to provide accurate advice and support.
- Gather and report market intelligence, customer feedback and industry insights to support business development and strategic decision-making.




- Ensure accurate, confidential and compliant client record management across relevant systems.

- Strong understanding of community resources and referral networks.
- Ability to work across teams to achieve business objectives and KPIs.
- Skilled at identifying and recommending process and service improvements.

Essential Requirements

- Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
- Relevant probity checks required by legislation and WMQ Limited policy.
- Current Driver’s Licence and willingness to travel for work
- Demonstrate behaviours aligned with the organisation's Model of Care, Charter, Vision, Mission and Values while adhering to policy, process and legislative requirements.
