We are seeking a dedicated Practice Nurse to join our established medical practice in Wyoming, NSW. This role offers the chance to focus on Chronic Condition Management, preventative healthcare, patient education, and coordinated care within a supportive and collaborative team environment.

About the Role

n

Key responsibilities include:

n

n




- Developing and reviewing Chronic Condition Management Plans
n
- Conducting health assessments and preventative health activities
n
- Providing patient education and lifestyle support
n
- Monitoring patient progress and coordinating follow-up care
n
- Working closely with GPs, specialists and allied health providers
n
- Identifying patients eligible for chronic disease management services
n
- Maintaining accurate clinical and Medicare documentation
n
- Supporting quality patient outcomes through collaborative care
n

About You

n

To be successful, you will have:

n

n
- Current AHPRA registration as an RN or EEN
n
- Experience in General Practice or Primary Healthcare (highly regarded)
n
- Knowledge of Chronic Condition Management and preventative health
n
- Strong communication and organisational skills
n
- A patient-centred approach and genuine passion for healthcare
n
- Ability to work independently and as part of a team
n
