Help older people live well at home with care that reflects their goals, preferences and changing needs.

We’re looking for a compassionate, organised Care Partner to join our Gold Coast Home Care team. You’ll build trusted relationships with participants and their families, coordinate personalised services, and support people to maintain their independence and make informed choices about their care.

About the role

You’ll manage a portfolio of participants, taking the time to understand what matters to each person. Your responsibilities will include:

- Developing and reviewing care plans that support wellness, reablement and independence.
- Coordinating services and maintaining regular contact with participants and their families.
- Helping participants understand their funding, budgets and available supports.
- Recognising changes in needs or risks and escalating clinical concerns appropriately.
- Coordinating complex supports, including restorative and end-of-life care, assistive technology and home modifications.
- Working with clinicians and care teams on reassessments, case conferences and ongoing care planning.
- Keeping accurate, timely records and meeting quality and compliance requirements.
- Providing direct care when needed to support participants and the service.

Throughout your work, you’ll promote dignity, culturally safe care, informed choice and supported decision-making.

About you

You bring experience in care coordination, case management, home care,



aged care, health or disability services. You’re confident balancing the practical details of service coordination with the personal connection that makes good care possible.

You’ll also bring:

- Strong care planning, organisation and risk management skills.
- An understanding of Support at Home, CHSP, aged care quality standards and participant rights.
- The ability to recognise changing or complex needs and seek the right support.
- Clear communication, sound judgement and a collaborative approach.
- Confidence working with participant budgets, digital care management systems and Microsoft Office.

Experience with My Aged Care, dementia, complex ageing or palliative care will be highly regarded.

Qualifications and requirements

- Certificate III in Individual Support or equivalent.
- Current First Aid Certificate.
- Relevant medication support and healthy body systems competencies.
- Current driver’s licence and access to a comprehensively insured vehicle (mileage reimbursed)
- Current National Police Certificate, or willingness to obtain one.
- Ability to meet relevant organisational vaccination requirements.
