Melbourne City Mission (MCM) is seeking a Case Manager to support young people at risk of homelessness in the Western and Inner Metro Melbourne area. You will work with young people aged 12-18, or 12-21 CALD backgrounds, to identify goals and stabilise housing outcomes, through the Reconnect framework.

You will deliver community development, outreach, and group work initiatives, with a strong focus on early intervention, reporting, and team-oriented partnerships with schools and service
