UnitingCare Queensland is seeking a Supported Living Facilitator for the FaDS team in Ipswich. The role focuses on assisting individuals with daily living, personal care, and autonomy within a supportive team workplace.

You will collaborate with families, ensure respect and confidentiality, and contribute to individual plans that align with the person’s life aspirations. A current NDIS check and a valid driver's license are required, with regular sleepover and weekend shifts.
