Melbourne City Mission is seeking a Case Manager to support young people at Circuit Breaker Refuge. You will work with Youth Refuge Workers and a multidisciplinary team to deliver high-quality, evidence-based case management and care coordination.
The role focuses on housing stability, safety planning, and pathways out of homelessness, with prospects to lead care planning and liaise with external providers. A relevant bachelor degree is required and experience in refuge settings is preferred.
