CenterWell Home Health seeks a Home Health Physical Therapist to help patients regain strength, mobility and independence in their own homes. You will deliver prescribed therapy, assess progress, and document outcomes to guide physician decisions and support the care team.

Reporting to the Clinical Manager, you will train PTAs and Home Health Aides, ensure explicit care instructions, track progress, and coordinate discharge summaries.
