BissyCare is seeking a Disabilities Service Officer to join its team to deliver the following responsibilities:
Developing and implementing individualised care plans tailored to the specific needs of residents.
Assessing community requirements for health, welfare, housing, employment, training, and other services.
Monitoring and reporting on residents' physical and emotional health, ensuring any concerns were promptly addressed with relevant healthcare specialists.
Assessing community requirements for health, welfare, housing, employment, training, and other services.
Engaging with families, welfare agencies, government organisations, and private businesses to identify and address pressing community issues.
Building robust partnerships to ensure a coordinated approach to community support.
Raising awareness about the resources and services available to individuals and families within the community.
Ensuring that stakeholders are informed about how to access these resources effectively.
ABOUT YOU
Minimum Diploma in Community Services, Individual Support, Mental Health or related subject
Experience with mental health & behavior is highly desirable
Ability to use technology
Passionate about helping people
Excellent communication skills (written & verbal)
Excellent documentation skills
Must have desire and commitment to ongoing learning, including the need to gain more knowledge and skills to improve their practice.
