Job Description
Align Care is seeking a dedicated Support Worker to assist NDIS participants in understanding and effectively utilising their plans in ACT.
You will connect participants with NDIS, community and mainstream services, and coordinate supports with families, providers and allied health professionals.
You will monitor plan implementation, identify gaps and assist with reassessments, while maintaining accurate case notes and required documentation.
