Social Worker - Cancer Care Chronic Disease
Join the Cancer Care and Chronic Disease team at the Royal Brisbane and Women's Hospital and help make a meaningful difference to the lives of patients and families navigating cancer treatment and chronic health conditions.
This role provides the opportunity to deliver high-quality social work services within a large tertiary hospital, supporting individuals and families through some of the most challenging periods of their healthcare journey.
Working across inpatient and outpatient settings, you will provide psychosocial assessment, risk management and evidence-based interventions that support positive patient outcomes and continuity of care.
The role offers exposure to a diverse and complex caseload, including patients affected by life-threatening illness, grief and loss, while working within a highly skilled multidisciplinary team committed to delivering compassionate, patient-centred care.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Deliver high-quality social work services through comprehensive psychosocial assessment, risk identification and intervention for patients affected by cancer and chronic disease.
Support patients and families experiencing complex issues including grief and loss, adjustment to life-threatening illness, domestic and family violence, child protection and elder abuse concerns.




Work collaboratively within multidisciplinary teams to coordinate care, facilitate safe discharge planning and optimise psychosocial outcomes across inpatient and outpatient settings.
Contribute to clinical education, professional development and service improvement initiatives that enhance patient care and strengthen social work practice.
Build positive relationships with patients, carers, colleagues and external agencies while demonstrating excellent communication, advocacy and teamwork skills.
