About the Role
In this role, you will:




Prepare and coordinate Chronic Condition Management Plans and Reviews
Support patients in managing chronic health conditions and achieving positive health outcomes
Conduct health assessments and preventative health activities
Work closely with GPs and the wider multidisciplinary healthcare team
Provide patient education and ongoing support
Maintain accurate clinical records and appointment administration
About You
To be successful in this role, you will have:
Current AHPRA registration as a Registered Nurse or Enrolled Nurse
Previous experience in Chronic Condition Management (highly regarded)
Solid communication and interpersonal skills
Excellent organisational and time management abilities
A proactive, patient-focused approach to care
The ability to work both independently and collaboratively within a team
