St Vincent's Hospital Sydney is seeking a Clinical Nurse Specialist (Grade 2) to lead the centralised intake function within Chronic and Complex Care Service in Darlinghurst. You will coordinate referrals, triage and service access with multidisciplinary teams to ensure timely, patient-centred care.

The role requires a NSW RN registration, post-graduate qualifications, and at least three years' experience, including chronic heart failure or cardiac care.
