UnitingCare Children, Young People and Families is seeking a Clinical Care Partner to join our team serving Bowen, Proserpine and surrounding areas. You will oversee and coordinate complex clinical and care needs, develop care plans, manage budgets, and liaise with multidisciplinary teams to deliver person-centred care across the community.

The role requires current AHPRA registration or equivalent Allied Health qualified, Australian driver's licence, and ability to work autonomously.
