Home in Place is seeking a compassionate Tailored Support Coordinator to join our Community Participation team on the Central Coast.
The role focuses on assessments, planning and tailored support to occupants in social and affordable housing, helping them access services to improve wellbeing and independence.
You will work with tenants, community organisations and internal teams to promote social inclusion, education, employment and long-term positive life outcomes.
