Darling Downs Health is seeking a Clinical Nurse - CHIP in Kingaroy to support safe transitions from hospital to home through expert care coordination and clinical leadership. You'll work with patients, families and partners to achieve better health outcomes.
You'll coordinate personalised care plans, build solid relationships with clinicians and community partners, and use advanced clinical judgement to improve continuity of care.
