Job Description
East Grampians Health Service invites a part-time Carer Support Program Coordinator to join Community Services - EGHS @ Home in Ararat.
You will identify carers' needs, respond with suitable services, and facilitate informal group meetings, advocating for carers and expanding the program through referrals.
Reporting against funding targets and working within the EGHS@Home framework, you'll support carers with compassionate, confidential service delivery and solid time management in a
