Job Description
Providence Disability and Ageing Support Services – Crows Nest NSW
n
Ensure the team complies with the Disability Services Act Standards, NDIS framework and other relevant legislation.
n
Apply person-centred approach and solution-focused, and strengths-based approach
n
Coordination and development of clients' care plans in liaison with families and other stakeholders.
n
Maintaining of participants records and providing updates on progress in weekly, monthly or quarterly reports.
n
Ensuring high-quality service delivery that aligns with individuals' needs and goals.
n
Ensure a high level of participants satisfaction measured through regular scheduled feedback and review processes.
n
Coordinate and oversee rostering and timesheet requirements according to client support needs.
n
Assisting young people to solve social, emotional and financial problems.
n
Assist clients with daily living activities, such as personal care, meal preparation, household tasks and maintaining routines.
n
Support clients with appointments, transport and community access.
n
liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
n
Development of skilled relationships with the participants, their families and other stakeholders and engagement and regular stakeholder meetings.
n
Provision of accurate and timely case notes, incident reports and house reporting.
n
Assessing clients' needs and planning, developing and implementing educational, training and support progress.
n
Coordination and management of day-to-day operations in our Supported Independent Living program.
n
Interviewing clients and assessing the nature and extent of difficulties
n
Contributing to the development and implementation of service improvements.
n
Conducting regular visits to clients home and community locations.
n
Assessing community need and resources for health, welfare, housing, employment, training and support programs.
n
Provide pre-planning support for new clients and families/carer transitioning to the agencies that can provide additional help.
n
Facilitating communication and collaboration between different teams and departments.
n
Developing and reviewing the client support plan
n
Continue as the point of contact, identifying clients' needs and ensuring they are communicated with regularly, and this is documented prior to formal admission to the service.
n
Monitoring and auditing clients' files to ensure accuracy and compliance.
n
Identify and report risks, abuse, neglect or incidents in accordance with legislation and workplace policies.
n
Develop positive, respectful relationships with clients and promote their independence, choice and dignity
n
Monitor and report changes in clients' health, behaviour, wellbeing or support needs.
n
Encourage development of life skills, including communication, decision-making, budgeting and independent living skills.
n
Respond appropriately to challenging behaviours or complex support needs, using approved strategies.
n
Work collaboratively with families, carers,
support coordinators, nurses, allied health professionals and other service providers.
n
Maintain confidentiality, privacy, safeguarding and professional boundaries.
n
Participate in team meeting, training, supervision and ongoing professional development.
n
The successful applicant will demonstrate:
n
n
Certificate III, IV or degree in Disability, Diploma of Community Service or other relevant qualification.
n
Knowledge of the NDIS Quality and Safeguarding Framework.
n
NDIS knowledge and awareness.
n
Extensive knowledge regarding relevant legislation in the disability sector.
n
Experience working in the Disability and Community setting with an accommodation facility supporting clients with high/complex needs.
n
Experience supporting people with high support needs and personal care.
n
Experienced supporting people with disability and behaviours of concern utilizing an empathetic and person-focused approach.
n
Demonstrated capacity to manage crisis and critical issues and deal with complex and challenging situation.
n
Demonstrated capability in risk identification, management, documentation and escalation.
n
Strong Coordination, problem-solving, and leadership skills.
n
The ability to contribute positively within a team.
n
Experience developing NDIS plans, assessments or equivalent participant-facing documentation.
n
Ability to use computers, tablets and smartphones
n
n
Requirement:
n

NDIS Worker Screening Check and NDIS Module
n
Valid First Aid Certificate with current CPR
n
Current Working With Children Check (WWCC)
n
A current unrestricted NSW Driver License with comprehensive insurance
n
