The Opportunity
n
As a Care Coordinator, you'll be the engine behind outstanding care delivery. You'll take ownership of client services, support-worker coordination and day-to-day service outcomes.
n
Cantonese or Mandarin language skills are highly regarded, as this role supports clients and families who may prefer to communicate in these languages.
n
What You'll Do
n
n
Coordinate client bookings and service requests accurately and efficiently.
n
Build rosters that meet client needs, budgets and SCHADS requirements.
n
Match clients with suitable support workers based on skills, preferences and availability.
n
Keep clients, families, support workers and internal teams informed.
n
Respond to roster changes, incidents and complaints with calm confidence.
n
Maintain accurate records and follow through until issues are resolved.
n
Build trusted relationships that support positive client outcomes.
n

About You
n

You'll bring:

Experience in aged care or disability, including exposure to the Support at Home program or home-care service delivery.
n
A relevant qualification in Health, Aged Care, Disability or Community Services.
n
Strong communication and relationship-building skills.
n
The ability to solve problems and make sound decisions under pressure.
n
Excellent organisation and attention to detail.
n
Confidence managing competing priorities in a fast-moving setting.
n
Knowledge of care plans, client budgets and the SCHADS Award will be highly regarded.
