We're excited to welcome a Social Worker Senior to our Social Work Acute Care Service Team based at Sunshine Coast University Hospital, Birtinya.

The Geriatric Evaluation and Management and Hospital in the Home (GEM / HITH) Team provides high-quality social work services to often older patients following discharge from hospital, through a home-visiting service. Experience in therapeutic interventions relating to adjustment to a new diagnosis, anxiety management, dementia support, carer support, discharge planning, and complex grief and loss counselling would therefore be highly desirable.

As a highly functioning multidisciplinary team member, deliver high level Social Work clinical services within the home, to
stakeholders/customers
of the Sunshine Coast Hospital and Health Service and Queensland Health (SCHHS).

Key Responsibilities include:

Deliver independent high level social work clinical practice with infrequent need for direct clinical practice supervision. Exercise professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence.

Provide high level clinical advice, education and influence towards a common patient-centred vision and direction to professional operational teams and relevant stakeholders regarding service delivery.

Provide leadership oversight and clinical practice supervision to less experienced social work practitioners and students as delegated.

Adhere to relevant expectations outlined in the SCH and Allied Health Governance Framework documents and Business Rules to ensure that best patient outcomes are achieved in a transparent, ethical and accountable manner.

Remuneration based pro rata on equivalent full time value up to $******* p.a., comprising salary rates: $***** - $***** p.h., employer contribution to superannuation ******%) and annual leave loading (17.5%) (HP4) (Fixed term temporary part time position available until June ****, 30.4 hrs p.f.).
