HammondCare is hiring a permanent part time Clinical Partner - Nursing to support clients in the Blue Mountains region.
You will perform home visits, assess needs, set goals, and deliver tailored nursing care across multiple HammondCare locations.
As a Registered Nurse with AHPRA registration, you will work within a multidisciplinary team, travel between client homes with your own vehicle, and contribute to high-quality, relationship-based care for older adults living at home.
