Based in Werribee Mercy Hospital, the role supports clients in community and outpatient settings, delivering psychosocial assessments, care coordination and advocacy.
You will work Monday–Friday with no weekend or public holiday shifts, joining a high-performing, multidisciplinary team to aid access to services and deliver patient-centred care.
