Job Description
Service Delivery Coordinator - Disability Supports
n
Comfort N Care Support Qld Pty Ltd – Toowoomba QLD
n
People person. Problem solver. Paperwork wrangler.
n
Are you an experienced NDIS professional who loves making sure participants receive great support, staff know what they're doing, and the paperwork is actually up to date?
n
Do you enjoy coordinating the moving parts of disability service delivery, building strong relationships and helping a team deliver its best work?
n
Comfort N Care Support QLD is looking for a Service Delivery Coordinator to join our growing team in a newly created, permanent part-time position.
n
This is a hands‐on role for someone who knows the disability sector, enjoys taking ownership of their work and can bring a calm, organised approach to a busy service environment.
n
About the role
n
Working closely with our Directors and rostering team, you'll coordinate the participant side of our services and help ensure supports are safe, person‐centred, consistent and delivered by appropriately trained staff.
n
You'll be a key point of contact for participants, families, support workers and external stakeholders, while helping maintain the quality systems and documentation that keep our services running smoothly.
n
What you'll be doing
n
n
Coordinating participant onboarding, service delivery, reviews and ongoing satisfaction.
n
Maintaining service agreements, support plans, risk assessments and participant documentation.
n
Supporting the implementation of Positive Behaviour Support Plans (PBSPs) and ensuring staff understand participant‐specific requirements.
n
Identifying training needs and organising staff training, including manual handling, medication assistance, seizure management and other specialist supports where applicable.
n
Monitoring staff competencies and ensuring required training and documentation remain current.
n
Providing day‐to‐day guidance, supervision and support to disability support workers.
n
Working with rostering to ensure participants are matched with suitable and appropriately trained staff.
n
Following up participant feedback, complaints, incidents and service delivery concerns.
n
Liaising with families, Support Coordinators, allied health professionals and other stakeholders.
n
Assisting with quality improvement, compliance and the development of practical service delivery processes.
n
Escalating complex matters and significant operational decisions to the Directors.
n
n
What we're looking for
n
You'll ideally have:
n
n
Demonstrated experience in disability service delivery, team leadership or a senior coordination role.
n
Strong knowledge of the NDIS and person‐centred support.
n
Practical experience working with Positive Behaviour Support Plans.
n
Experience supporting or supervising disability support workers.
n
Confidence coordinating training and participant‐specific competency requirements.
n
Strong organisational skills and attention to detail.
n
Experience with support plans, risk assessments, incidents and participant documentation.
n
Excellent communication skills and the ability to build positive relationships with participants, families and staff.
n
A proactive,
solutions‐focused approach and the ability to manage competing priorities.
n
A genuine commitment to participant choice, dignity, safety and quality of life.
n
n
Experience with SIL, complex support needs and NDIS quality systems will be highly regarded.
n
The kind of person we're looking for
n
You're the person who follows things through. You can organise a training session, support a worker, update a risk assessment and return a family's phone call without needing to be reminded five times.
n
You're approachable, skilled and comfortable having difficult conversations when needed. You understand that good service delivery is about both people and processes—and that neither works particularly well without the other.
n
Most importantly, you care about getting things right for participants.
n
Why join Comfort N Care?
n
We're a growing, locally operated disability support provider with a genuine focus on quality, relationships and doing the right thing by the people we support.
n
This newly created role offers the opportunity to:
n
n
Work directly with the Directors and contribute to how our services are delivered.
n
Enjoy a varied role with genuine responsibility and the opportunity to make a difference.
n
Help develop and improve practical service delivery systems.
n
Be part of a supportive team that values initiative, accountability and a sense of humour.
n
Enjoy a permanent part‐time arrangement across three days per week.
n
n
Location: Toowoomba, Queensland
n
Employment Type: Permanent Part-Time – 3 days per week
n
Remuneration: $***** per hour plus superannuation, subject to the applicable SCHADS Award classification and pay point.
