The Role

Danny Met Sally Support Coordination is growing, and we are looking for a dedicated and empathetic NDIS Support Coordinator to join our journey and purpose of delivering premium quality services to our Participants.

This role is being offered full time, remote/working from home and can be based anywhere in Brisbane or the surrounding region.

Key Responsibilities

- Building a genuine rapport and maintaining relationships with each Participant

- Support participants to understand and implement their NDIS plans.

- Coordinate and monitor a range of supports and services to ensure participant goals are achieved.

- Assist participants to build their capacity to navigate the NDIS and broader community systems.

- Advocate for participants when accessing services, housing, health, mental health, and community supports.

- Develop and maintain strong relationships with participants, families, guardians, service providers, and government agencies.

- Conduct regular reviews of participant goals, support arrangements, risks, and funding utilisation.

- Prepare reports, case notes, funding reviews, and evidence for NDIS plan reassessments.

- Facilitate stakeholder meetings and collaborative planning discussions.

- Identify and respond appropriately to safeguarding concerns, critical incidents, and emerging risks.





- Support participants experiencing complex circumstances including psychosocial disability, challenging behaviours, homelessness, hospital discharges, justice involvement, and crisis situations

- Maintain accurate records in accordance with organisational policies, NDIS Practice Standards, and privacy requirements.

- Work independently while managing a diverse caseload and competing priorities.

About You

You will be passionate about supporting people with disabilities, promoting positive change, and creating a culture of empowerment.

Key requirements:

- Minimum 2 years as a Support Coordinator or Specialist Support Coordinator experience is essential.

- Tertiary qualification in Social Work, Human Services, Community Services, Psychology, Disability, Mental Health, or a related field.

- NDIS Worker Screening Check (Yellow Card)

- Working with Children Check (Blue Card)

- Drivers’ License and own reliable transport

- Demonstrated experience supporting participants with complex needs, including psychosocial disability, mental health conditions, challenging behaviours, homelessness, justice involvement, substance misuse, and complex family dynamics.

- Experience working collaboratively with multidisciplinary teams including Allied Health, Behaviour Support Practitioners, SIL providers, hospitals, guardians, and government agencies.

- Proven ability to manage crisis situations, risk management, safeguarding concerns, and complex service coordination.

- Strong understanding of NDIS legislation, Practice Standards, and participant safeguarding requirements.
