About the Role

Support to Thrive is seeking a motivated and experienced Support Coordinator based in Hervey Bay or surrounding areas. This is a rewarding chance to work amongst a dynamic and supportive team of individuals with diverse backgrounds and strengths. This role is permanent part-time for 20-25 hours per week with the opportunity to increase to full-time.

As a Support Coordinator, you will play a key role in supporting participants to navigate the NDIS, develop capacity and to connect them with the right supports to achieve their goals. We pride ourselves on building strong relationships, thinking creatively and maintaining a high standard of practice across all aspects of our work.

As a multi-service provider of other therapies, we also encourage Social Work, Psychology and Counselling students to apply. Support to Thrive is committed to helping their team to work towards higher qualifications and can provide support for those looking to enter the Positive Behaviour Support workforce with the appropriate educational and experienced backing.

In this role, you will:

Support participants to understand and implement their NDIS Plans





Connect participants to appropriate supports, service providers and community resources

Build participants' capacity to coordinate their own supports and make informed decisions

Work collaboratively with participants and their families, carers and other stakeholders

Monitor supports and ensure services are meeting participant needs and goals

Maintain accurate records and case notes in line with organisational and NDIS requirements

Prepare and provide requested reports to the NDIA

Identify and respond to risks, barriers or changes in participant circumstances

To be successful in this role, you will:

Have minimum 12 months experience in Support Coordination, or similar experience in the above-mentioned requirements

Excellent communication and problem-solving skills

Ability to work independently while also contributing to a team environment

A genuine passion for empowering people with a disability

Current driver's licence and reliable vehicle (with comprehensive insurance)

NDIS Worker Screening Check (or willingness to obtain prior to employment)

Working with Children Check (or willingness to obtain prior to employment)

Although not essential, relevant qualifications in Community Services, Disability or Individual Support are highly desirable
