Portland District Health is seeking an Administration Officer - Service Provision for the Community Aged Care Services team. The fixed term, part-time role will support health care workers and coordinate services to assist clients to remain at home under CHSP guidelines.

You will collaborate with multidisciplinary teams, demonstrate solid communication and ICT skills, and contribute to person-centred care with a focus on delivery of high-quality community-based services.
