Care Navigator - Recovery Support/Outreach

An exciting opportunity for Mental Health Clinicians to join our Recovery Support Program! This is a permanent, full-time opportunity, coordinating post discharge outreach support following an inpatient admission.

About the Role

In this role you will be instrumental in facilitating care and support for our outpatients as part of a multidisciplinary team. You will need to have demonstrated confidence in the ability to engage patients across age groups and a variety of mental health diagnoses including mood disorders, addiction and eating disorders.

Reporting to the Director of Clinical Services, you will perform duties in collaboration with the multidisciplinary team to ensure a high level of quality patient care is delivered in line with the care values and strategic direction of the Hospital and Ramsay Health Care. Coordinating the outreach appointments, community visits and reporting upon the departmental key deliverables to the Executive team.

About You

You will have the ability to plan and coordinate individualised mental health with a commitment to excellence in the delivery of healthcare services to patients seeking mental health treatment. To succeed in this role, you will be a motivated and flexible team player with a high degree of professionalism,demonstrated leadership attributes and excellent communication skills.

You will need to demonstrate:

- Qualified as an Occupational Therapists (Mental Health), Social Worker (Mental Health), Psychologist, Counsellor/Therapist or Registered Nurse (registered with the appropriate governing body)
- Deliver individualised clinical follow up to patients post discharge aligned to their developed Mental Health plan whilst an inpatient




- Excellent written and verbal communication skills
- Ability to be self-motivated and function autonomously whilst also being able to work effectively as part of a multidisciplinary team
- Excellent time management and organisational skills
- Able to plan, organise and prioritise workload in a complex therapeutic environment
- Familiar with Microsoft Suite
- Have an unrestricted NSW drivers’ licence
- Have a personal vehicle that may be used for work
