Job Description

Sourcify Pty Ltd Key Responsibilites: Interview participants and assess their individual needs, circumstances, goals and barriers to accessing community services. Plan, develop and implement tailored support, education and community participation programs that promote participant independence and social inclusion. Monitor participant progress against agreed goals, maintain accurate case notes and prepare progress reports.

Identify appropriate health, housing, welfare, employment, training and recreational services available within the community. Refer participants and families to relevant external agencies where additional or specialised assistance is required. Liaise with families, community organisations, welfare agencies, government bodies, healthcare professionals and other service providers to coordinate participant support.

Support participants with disabilities to access community facilities, social activities, educational programs and other community-based services. Identify gaps in community resources and contribute to initiatives that improve access and outcomes for participants. Promote awareness of available community resources, disability services and support programs.

Assist with preparing service reports, funding-related submissions and documentation for government bodies and partner organisations. Requirements: Australian Citizens and Permanent Residents encouraged to apply An AQF Associate Degree, Advanced Diploma or Diploma in Community Services, Community Development, Welfare, Social Services or a closely related field. A minimum of 3 years of relevant experience in community services, disability services, welfare support, community development or a related setting.





Demonstrated experience assessing client needs, developing support programs, monitoring outcomes and coordinating referrals. Knowledge of community resources and service systems relating to disability, health, welfare, housing, employment and social participation. Ability to liaise effectively with participants, families, community organisations, government agencies and multidisciplinary service providers.

Strong written communication skills, including the ability to maintain case notes and prepare participant progress and service reports. Strong interpersonal, organisational, problem-solving and stakeholder engagement skills. Ability to work independently and collaboratively within a multidisciplinary service setting.

Demonstrated cultural sensitivity and commitment to inclusion, participant choice, dignity and social justice. Understanding of the NDIS and person-centred support practices is highly regarded.
