The Salvation Army in Hobart is seeking a part time Case Manager to provide rapid, person-centred support to older people who are homeless or at risk of homelessness. You will help clients access aged care, housing, and essential community services.

Based with the Care Finders program, you will navigate complex systems, work with the Care Finders team, and maintain clear case notes and regular updates to support positive outcomes for vulnerable clients across Launceston and beyond.
