Key responsibilities

- The successful candidate(s) will be responsible for reviewing and dispensing medications, conducting medication reconciliation and clinical reviews, providing expert medication advice to clinicians and patients, supporting discharge planning, and promoting the safe and quality use of medicines across the hospital.
- Working within a multidisciplinary healthcare environment, you will play a key role in medication safety initiatives, patient education, and the continuous improvement of pharmacy services.

What you need





- This position is responsible for facilitating and promoting patient safety and high-quality care while practising as a Pharmacist in accordance with the Pharmacy Board of Australia Registration Standards, Codes and Guidelines, including the Code of Conduct for Registered Health Practitioners, as well as South Metropolitan Health Service (SMHS) policies and procedures.
- The ideal candidate will hold current AHPRA registration, demonstrate robust clinical and communication skills, and have a commitment to delivering exceptional patient-centred care within a hospital setting.
- A statement addressing the suitability criteria, and specifically the Essential Criteria in the JDF in no more than 2-3 pages.
