BlueCare in Queensland is seeking a Clinical Care Partner to coordinate complex clinical and care needs for community clients across Bowen, Proserpine and surrounding areas. You will assess, plan and coordinate services to deliver person-centred, high-quality care funded through aged care programs.

You will spend most days on the road engaging with clients, developing and reviewing care plans, managing budgets, and liaising with clinical teams to respond to changing needs.
