Disability and Aged Care Support Workers

You will provide support to NDIS participants and Support at Home aged care participants in their homes and communities. Depending on your experience and qualifications, your work may include personal care and assistance with daily living, domestic assistance and household cleaning, community access, appointments and social activities, meal preparation and shopping assistance, transport support, medication assistance within your training and authority, capacity‑building and supporting participant goals, following care plans, clinical instructions and risk‑management strategies, and completing accurate progress notes and required documentation.

Key responsibilities

- Personal care and assistance with daily living
- Domestic assistance and household cleaning
- Community access, appointments and social activities
- Meal preparation and shopping assistance
- Transport support
- Medication assistance within your training and authority
- Capacity‑building and supporting participant goals
- Following care plans, clinical instructions and risk‑management strategies
- Completing accurate progress notes and required documentation

About you

- Experience providing disability, aged care or community‑based support
- A Certificate III or IV in Individual Support, Disability, Ageing Support or a related qualification, or relevant demonstrated experience
- A respectful, person‑centred approach
- Strong communication and interpersonal skills
- The ability to build appropriate professional relationships with participants, families and other professionals
- Confidence working independently and as part of a broader care team
- The ability to follow support plans,



policies and workplace instructions
- Good written skills and the ability to complete progress notes accurately
- Reliability, professionalism and a genuine commitment to the care sector
- A welcoming, inclusive and respectful approach to people from diverse backgrounds

Mandatory requirements

- A current NDIS Worker Screening Clearance
- A current National Police Check suitable for aged care services
- A Working with Children Check (Blue Card), where required
- Current First Aid and CPR certification
- Completion of the NDIS Worker Orientation Module
- Access to a reliable, registered and appropriately insured vehicle
- Evidence of relevant qualifications and training
- The physical capacity to safely complete the inherent requirements of the position
- Pay aligned with the applicable SCHADS Award Home Care Employee rates, based on your duties, qualifications, skills and experience
- Applicable penalty rates, travel and vehicle allowances
