Care Connect is seeking a customer-focused Intake Coordinator to join our Client Services team in Australia. You will be the first point of contact for clients, families and referral partners, connecting people with information, services and support from their very first interaction.

You will gather information, assess needs, coordinate referrals and schedule assessments while delivering person-centred support and maintaining accurate records in a rapid-paced environment.
