Lead care that helps people live well at home

Right at Home Brisbane West helps people stay independent and live well in their own homes. We're growing, and we need an experienced Clinical Care Manager to lead the clinical and operational side of our home care and community services.

This is a senior role with real influence. You'll set the standard for clinical care, lead a committed team, and shape how we look after every client.

About the Role

You'll run the day-to-day clinical and operational side of the business, working closely with the Branch Director and management team. Your work will include:

Clinical leadership and quality

Giving clinical leadership, guidance and support to Care Coordinators, Personal Care Workers and other care staff

Making sure every client receives safe, high-quality and individualised care

Overseeing assessments, care plans, reviews and changes in care needs

Managing clinical incidents, risks and complaints through to resolution

Keeping clinical governance, documentation and quality assurance to a high standard

Compliance and stakeholders

Ensuring compliance with Aged Care, NDIS and other regulatory requirements

Building strong relationships with clients, families, support coordinators and health professionals

People and operations

Supporting staff performance, training, mentoring and career development

Helping with recruitment, onboarding and workforce planning

Monitoring service delivery, client satisfaction and operational performance

Finding and putting in place better systems, processes and ways of delivering services

About You

You're a confident leader who can manage people and operations.



You understand what it takes to deliver excellent care in clients' homes and in the community.

You'll bring:

At least 5 years' experience as a Clinical Care Manager, Clinical Coordinator, Care Manager or in a similar senior role

A strong background in Home Care, Community Services or a similar care setting

A relevant clinical qualification, ideally Registered Nurse with current AHPRA registration, or another suitable health qualification

A solid understanding of the Aged Care and NDIS environments

Excellent clinical assessment and care planning skills

Experience in incident management, risk management and clinical governance

A record of managing day-to-day operations in a care organisation

You're also:

A capable leader and people manager who communicates clearly and warmly

Organised, detail-focused and thorough with documentation

Practical and focused on solving problems

Comfortable working on your own while contributing to a wider management team

Genuinely committed to person-centred care
