Job Description
Prevail Care in Sydney, NSW, seeks a Care Liaison Manager to own the relationship with the NDIS ecosystem, including Support Coordinators, Plan Managers, LACs, and NDIA planners.
This role requires established Sydney networks and the ability to bring referrals while managing intake and onboarding for recent participants.
You will develop person-centred plans with participants and families, monitor delivery, and ensure compliant documentation.
