Job Description
ForHealth Group's Forest Hill practice is seeking an experienced Part time Chronic Disease Management (CDM) Nurse to support GPs and patients in preventing hospital admissions.
You will prepare care plans, empower patients to take ownership of their health, and collaborate with a multidisciplinary team to improve health outcomes in the local community.
AHPRA registration is required, with solid assessment and communication skills.
