You will support patients and families with psychosocial assessments, discharge planning, and risk management within a team-oriented team.
The role offers flexibility for weekday and weekend shifts, mentoring for Grade 1 social workers, and opportunities to contribute to high-quality, person-centred care in a agile healthcare setting.
