Job Description

HammondCare is seeking a Registered Nurse to join our Home Care Clinical Care & Practice Team, supporting clients living at home across the Penrith, Hawkesbury and Lower Blue Mountains region. This permanent part-time role covers three days per week, with travel between client homes using your own vehicle.

You will assess care needs, set goals, and provide treatment and interventions to help older adults stay independent and secure at home.
