In this role, you will assess individual client needs, plan and coordinate care, and provide clinical guidance to less experienced staff.

- Deliver proficient, person-centred palliative nursing care and case management across multiple community locations.
- Assess, plan, implement and evaluate care to achieve the best possible outcomes for clients and families.
