About the role

The Social Worker provides professional social work services as part of the multidisciplinary team within the Department of Social Work Services at Caboolture and Kilcoy Hospitals. The role supports inpatients, outpatients, families and carers experiencing acute psychosocial trauma, complex psychosocial issues or circumstances that impact their health and wellbeing, helping them achieve the best possible outcomes from healthcare services.

The successful candidate will deliver assessment, counselling and intervention services, contribute to multidisciplinary case management, and provide support across a range of acute clinical areas. The role also supports service improvement initiatives, professional education, research activities and the ongoing enhancement of patient-centred care across the Caboolture, Kilcoy and Woodford Directorate.

About you

- Provide social work assessment, counselling and intervention services for patients, families and carers with complex psychosocial needs.




- Contribute to multidisciplinary case management and patient care planning through evidence-based clinical practice.
- Deliver social work services across acute clinical areas, including Caboolture and Kilcoy Hospitals as required.
- Support the education and supervision of students and less experienced staff within the healthcare team.
- Participate in research, quality improvement and service development activities that enhance patient outcomes and service delivery.
