Central Highlands Rural Health in Victoria seeks a caring Care Partner to join our Home Support Program. You will work with older people and their families to coordinate services, support goal achievement and deliver a person-centred care experience that enables clients to live safely and independently at home.

The role is permanent part time, with hours negotiated. Salary is HS5 at 48.87 AUD per hour plus superannuation.
